For security purposes must present photo id at time of event.

In consideration for my attendance and participation in Premier Martial Arts, I, the student/parent, acknowledge the existence of certain inherent risk in this type of training and hereby agree to assume all risks. I further relieve the Premier Martial Arts, it's management, assigned staff and fellow students, from any and all liability resulting from personal injury or loss of personal belongings. I also hereby state that the students named above are physically fit to take the prescribed course of instruction and do so of their own free will. We hereby authorize Premier Martial Arts its agents, successors and assigns to photograph/video me or my family and/or use our voice without restriction and to utilize such photographs/video and/or voice transcriptions for any commercial purpose, including but not limited to the promotion and marketing of the School, and we shall not be entitled to received to receive any compensation whatsoever of any kind as a result of such use.
