Insurance/Medical/Accident Coverage Is NOT Provided By Solebury Township or the Summer Camp Program:

REGISTRATION FEES:
$250 PER PERSON PER WEEK

WITHDRAWAL FEE: $50 PER PERSON PER EACH WEEK UP UNTIL MAY 15. THERE ARE NO REFUNDS AFTER MAY 15.

REGISTRATION DEADLINE: Registration will continue until the maximum number of permitted camp spaces has been reached. Once that number has been reached, no exceptions can be made to allow additional campers into the program.

EQUIPMENT: Campers must bring their lunch and drinks to camp each day. Please use ice packs in your child’s lunch if it needs to stay cold. Campers should bring their own individual hand sanitizer and mask if desired.

PROGRAM POLICY: Wearing a face mask is optional • Parents and Guardians are required to sign in their child during drop off and check-in, and during pick-up. • Please have your child stay home if sick! Children with fevers or displaying obvious symptoms of severe respiratory illness will not be allowed to attend or remain in camp • Children are required to follow current CDC guidelines as practical, and must be able follow all directions from staff • Sharing of food, utensils, electronic devices, toys, books, games, and personal items, etc. is NOT permitted.

MEDICAL AND EMERGENCY TREATMENT AUTHORIZATION:
I hereby give my permission for Solebury Township and / or the Parks & Recreation Director, personnel or counselors to arrange for transportation to the appropriate medical facility and authorize emergency treatment as deemed necessary by a Physician or trained medical personnel for illness or injury to myself / to my child that has oncurred while participating in a Solebury Township Recreational Program. I understand that I will be notified immediately of the need for emergency transport or treatment, and if not available, treatment deemed necessary will be authorized.

CAMP, SPORTS, FIELD TRIP, ACTIVITY AUTHORIZATION, GENERAL RELEASE AND COVID-19 RELEASE:
I, in consideration of myself / my child being permitted to participate in a Solebury Township Camp and/or Recreational Program, on behalf of myself and / or my child, our heirs, personal representatives and assigns hereby release Solebury Township, Bucks County, PA, its Supervisors, Director of Parks & Recreation, agents, employees, officers, counselors, successors and assigns from all liability, actions, suits, and claims, including but not limited to, wrongful death, personal injury, accidents, illness, disease, negligence, and intentional torts, and hereby waive all such claims which may be raised by me, my child, my heirs, my personal representatives or assigns.

WAIVER AND RELEASE:
I am fully aware of and appreciate the risks, including the risks of catastrophic injury, paralysis, illness, disease and even death, as well as other damages and losses, associated with participation in a recreation camp program, sports event, and other activities. I agree on behalf of myself, and/or my child, our heirs and personal representatives, that Solebury Township, Bucks County, PA, its Supervisors, Director of Parks & Recreation, agents, employees, officers, counselors, successors and assigns shall not be held liable for any injury, loss of life or other loss or damage as a result of myself and/or my child's participation in said program. This Waiver & Release shall also be for the benefit of and run in favor of any contracted instructors employed or volunteering on behalf of Solebury Township and the Summer Camp.
