INTERNATIONAL
DAN GRADE CERTIFICATION
This "on-line" Black Belt Dan Grade Application is for all Martial Artists who wish to have their Taiho Jutsu Grade(s) recognized by and registered with the Martial Arts International Federation (MAIF) tm, regardless of rank, system, gender, ethnicity, or age.
In using this On-Line Application Form, it is Important to know that you are applying for "ITJ" Dan Rank Certification or Promotion.
ITJ Yudansha Rank Fees: Life Member - Any Black Belt Level - $150, Black Belt - $200, Shodan - $225, Nidan - $250, Sandan - $275, Yondan - $300, Godan - $350, Rokudan - $400, Shichidan - $450, Hachidan - $500, Kudan - $550, Judan - $600.
(Note: Life Members pay only a "flat fee” of $150.00 for Any Level of Dan Grade Certification or Promotion.)
If your application for Dan rank certification, validation or promotion is Not approved, the fee collected will be refunded to you less the processing fee.
Note: For ranks "below" Black Belt - You will need to provide your Sensei's name, phone number and email address so that training and rank can be confirmed. See On-Line Registration
For all "ITJ Dan Grades" of Black Belt - You are required to submit the following Documents, Photo, and Information to the MAIF International Office at maifhq@gmail.com or brucebethers@gmail.com
Submit -
1.
Copies of all your previous Rank Certificates or Documentation of Training.
2. Your
complete martial arts & professional bio or resume.
3. Your
current Photo (a "close-up"), Men - "Coat & Tie" and Women in "Business Attire".
4. Three
Letters of Recommendation.
5.
Complete Background Check
6. You are required to attend a National Seminar OR be evaluated by the MAIF Internationally Certified Instructor / Examiner OR submit a Video of yourself demonstrating intermediate & advance techniques from your system.
7. You
must be a current "Annual" or "Life Member" of MAIF.
NOTE - The Non-refundable Processing Fee for all Dan Grades in $25.00

Waiver of Liability -
In consideration of being allowed to participate in any way in the Martial Arts International Federation (MAIF) athletics/training program, and related events and activities, the undersigned:
Agree that the parent(s) or legal guardian(s) will instruct the minor participant that prior to participating, he or she should inspect the facilities and equipment to be used, and if the participant believes anything is unsafe, he or she will immediately advise his or her coach or supervisor of such condition(s) and refuse to participate.
Acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions or negligence but the actions, inactions or negligence of others, the rules of play, or the condition of the premises or of any equipment used. Further, that there may be other risks not known to us or not reasonably foreseeable at this time.
Assume all the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability or death.
Release, waive, discharge and covenant not to sue the Martial Arts International Federation, its officers, its affiliated clubs / schools, regional taiho jutsu training organizations, their respective administrators, directors, agents, coaches, and other employees of the organization, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessees of premises used to conduct the event, all of which are hereinafter referred to as ‘releasees’ from any and all liability to each of the undersigned, his or her heirs and next of kin for any and all claims, demands, losses or damages on account of injury including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the releasee or otherwise.
