REFUNDS

Payment is due in order to secure a spot for your child. Payment plans available on the payment page. There is a $100.00 processing fee for all refunds.
Everyday your child will need to bring a backpack with:

LUNCH

A bag lunch (non-perishable please) with as many snacks and drinks as your child will need throughout the day (no candy please). If you will be purchasing lunch from Bodies in Motion, some snacks will still be advised for your child.
Sunscreen (For the days we have outside field trips. Please put on your child before he/she arrives.)

CONSENT TO EMERGENCY FIRST AID & TRANSPORTATION

I hereby give permission that my child may be given emergency treatment by a staff member at Bodies in Motion. I also give permission for my child to be transported by car, ambulance, or Aid car to an emergency center for treatment, and agree to hold Bodies in Motion,LLC and its employees harmless.

CONSENT TO MEDICAL CARE & TREATMENT

In the event that I cannot be contacted immediately, medical of surgical treatment can be administered to my child in the case of an accident or emergency, as prescribed by a treating physician, and hold Bodies in Motion and its employees harmless.

FIELD TRIP & TRANSPORTATION PERMISSION

My child is permitted to participate in field trips, to the park, or any other activities that would involve taking the child outside of the daycare for his/her benefit in attendance at this facility. I understand that field trips are an important part of the Bodies in Motion curriculum. I also understand that every child will be placed in a child restraint seat or seat belt depending on age and weight defined by Idaho State law.

SUNSCREEN CONSENT

I hereby give Bodies in Motion to apply sunscreen in accordance with the directions for use on the container. I must supply Bodies in Motion with sunscreen in a original container and labeled with my child’s name. I release Bodies in Motion from any liability for administering these preparations. It is the policy of Bodies in Motion that each child come to the facility with sunscreen already applied. Bodies in Motion will reapply in the afternoon prior to going outside.

PERMISSION TO BE PHOTOGRAPHED

Occasionally Bodies in Motion, Inc. uses photo or videos of its students in print ads, websites, other marketing mediums, or in the camp itself. I understand that my child's likeness may be used in such advertising. The images will be used for Bodies in Motion, Inc. purposes only and will not be given or sold to outside companies or individuals.

ADMISSION CONSENT

I agree to cooperate with the general policies of the center, to perform the obligations of the parents or guardians set forth in this agreement, and to abide by the rules, regulations and manual promulgated and provided by the center. Checking the box indicates that I have read the terms of agreement. I hereby agree to all of the above.
