UNITED STATES TAIHO JUTSU (USTJ)

 DAN GRADE APPLICATION

This "on-line" Black Belt Dan Grade Registration is for all Martial Artists who wish to have their Taiho Jutsu(s) recognized by and registered with the UNITED STATES JU-JITSU FEDERATION (USJJF) ®, regardless of rank, system, gender, ethnicity, or age. 

In using this On-Line Registration Form, it is Important to know that you are applying for USTJ Dan Rank Certification or Promotion. 

USTJ Yudansha Rank Fees: Black Belt - $150, Shodan - $175, Nidan - $200, Sandan - $225, Yondan - $250, Godan - $300, Rokudan - $350, Shichidan - $400, Hachidan - $450, Kudan - $500, Judan - $550. 

(Note: Life Members pay only a "flat fee” of $150.00 for Any Dan Grade Level Certification or Promotion.) 

If your application for Dan rank certification, validation or promotion is Not approved, the fee collected will be refunded to you less the processing fee. 

Note: For ranks "below" Black Belt - You will need to provide your Sensei's name, phone number and email address so that training and rank can be confirmed.  See On-Line Registration 

For all "USTJ Dan Grades" of Black Belt - You are required to submit the following Documents, Photo, and Information to the USJJF National Office at natlhq.usjjf@gmail.com / brucebethers@gmail.com 

Submit -

1. Copies of all your previous Rank Certificates. 

2. Your complete martial arts & professional bio or resume. 

3. Your current Photo (a "close-up" in Business Attire). 

4. Three Letters of Recommendation. 

5. Complete NCSI On-Line Background Check

6. Safe Sport On-Line Training Certification

7. Concussion On-Line Training Certification 

8. CPR On-Line Training Certification

9. You are required to attend a National Seminar OR be evaluated by the USJJF Nationally Certified Instructor / Examiner OR submit a Video of yourself demonstrating intermediate & advance techniques from your system. 

10. You must be an "Annual" or "Life Member" of USJJF. 

NOTE - The NON-refundable Processing Fee for all Dan Grades in $75.00

Billing and Payment
Billing and payment processing for Businesses' events is performed by Member Solutions, Inc. ("MSI"), and this contract is specifically assigned to MSI for all such purposes. Customer knowingly and irrevocably authorizes MSI to charge electronically Customer's bank account and/or credit card for any and all amounts due for Customer's, and anyone registering under Customer's, attendance and participation in the Event. Any returned payment will be assessed a $25.00 fee. Any unsuccessful electronic transaction is subject to a $10.00 processing fee. MSI is not responsible for any bank fees incurred by Customer. The United States Ju-Jitsu Federation (USJJF) is The National Governing Body for Ju-Jitsu in the United States of America and is the USA National Member of the North American Jiu-Jitsu Union (NAJJU), Panamerican Jiu-Jitsu Federation (PJJF), World Ju-Jitsu (WJJ), and the Martial Arts International Federation (MAIF) with the purpose of unifying the All American Ju-Jitsukas in an atmosphere of mutual respect and cooperation. The terms of membership are that each individual member consent to waive the United States Ju-Jitsu Federation (USJJF) from all liabilities as indicated below.

Waiver of Liability -
In consideration of being allowed to participate in any way in the United States Ju-Jitsu Federation (USJJF) athletics/sports program, and related events and activities, the undersigned:
Agree that the parent(s) or legal guardian(s) will instruct the minor participant that prior to participating, he or she should inspect the facilities and equipment to be used, and if the participant believes anything is unsafe, he or she will immediately advise his or her coach or supervisor of such condition(s) and refuse to participate.
Acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions or negligence but the actions, inactions or negligence of others, the rules of play, or the condition of the premises or of any equipment used. Further, that there may be other risks not known to us or not reasonably foreseeable at this time.
Assume all the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability or death.
Release, waive, discharge and covenant not to sue the United States Ju-Jitsu Federation, its officers, its affiliated clubs, regional sports organizations, their respective administrators, directors, agents, coaches, and other employees of the organization, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessees of premises used to conduct the event, all of which are hereinafter referred to as ‘releasees’ from any and all liability to each of the undersigned, his or her heirs and next of kin for any and all claims, demands, losses or damages on account of injury including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the releasee or otherwise.
