The Merced MMA Summer Wrestling Camp is geared towards 7^th-12^th grade boys & girls and will be held Thursday, July 7^th, Tuesday, July 12^th and Thursday, July 21^st, 3:30pm-4:45pm.

1. I acknowledge that I have voluntarily agreed to participate in the Merced MMA Summer Wrestling Camp

2. I understand that these activities are inherently dangerous regardless of the level of supervision. Injuries may occur due to physical situations including a slippery floor surface, an overzealous partner, or a combination of any number of circumstances. It is clear to me that my voluntary participation in any activity will expose me to risk of injury, and that any amount of time spent engaging in this activity only makes it more likely that injuries will occur at some time.

3. I AM VOLUNTARILY PARTICIPATING IN THESE ACTIVITIES WITH KNOWLEDGE OF THE DANGER INVOLVED AND HEREBY AGREE TO ACCEPT ALL RISKS OF INJURY OR DEATH

4. As consideration for being permitted to engage in these activities, and to use the facilities provided Oakdale, Merced, and Sonora Mixed Martial Arts Academy, I hereby agree that I will not demand, a claim against, or sue Oakdale, Merced, or Sonora Mixed Martial Arts Academy, its employees, or authorized agents for any injury or damage resulting from negligence or other acts, howsoever caused as a result of my participation in any activity and or instruction. I hereby release Oakdale, Merced, and Sonora Mixed Martial Arts Academy, its employees, and authorized agents from all actions, claims, and demands, that I now have or may hereafter have for injury or damage resulting from my participation in any activity at Oakdale, Merced, and Sonora Mixed Martial Arts Academy.

5. I hereby permanently waive any compensation whatsoever for the use of pictures, videos, media, coverage, etc. utilized by those associated with Oakdale, Merced, and Sonora Mixed Martial Arts Academy, which may be used for profit-making purposes.
