
USA-TKJ "CLASS - KYU" GRADE CERTIFICATION This "on-line" Class - Kyu Grade (below Black Belt) Registration is for all Martial Artists who wish to have their Judo rank(s) recognized by and registered with USA TRADITIONAL KODOKAN JUDO (USA-TKJ), regardless of rank, gender, ethnicity or age.
In using this On-Line Registration Form, it is Important to know that you are submitting an "Application" for Class - Kyu Grade Rank Certification, Validation or Promotion.
For all ranks "below" Black Belt - You will need to provide your Sensei's name, phone number and email address so that training and rank can be confirmed.
For all "Class - Kyu Grades" Belts - You are required to submit the following Documents, Photo and Information to the USA-TKJ National Office at natlhq.usatkj@gmail.com or brucebethers@gmail.com
Send by Email -
1. Copy of student's prior Class - Kyu Grade Certificate.
2. Student's Photo (a "close-up" in "Gi" or "Coat & Tie" or Business Attire).
3. Student must be a "Current or Life Member" of USA-TKJ

The USA Traditional Kodokan Judo (USA-TKJ) is The National Governing Body for Traditional Kodokan Judo in the United States of America and is the USA National Member of the Panamerican Judo Union (PJU) and the Martial Arts International Federation (MAIF) with the purpose of unifying the All American Traditional Judoka in an atmosphere of mutual respect and cooperation. The terms of membership are that each individual member consent to waive the USA Traditional Kodokan Judo (USA-TKJ) from all liabilities as indicated below.
Waiver of Liability -
In consideration of being allowed to participate in any way in the USA Traditional Kodokan Judo (USA-TKJ) athletics/sports program, and related events and activities, the undersigned:
Agree that the parent(s) or legal guardian(s) will instruct the minor participant that prior to participating, he or she should inspect the facilities and equipment to be used, and if the participant believes anything is unsafe, he or she will immediately advise his or her coach or supervisor of such condition(s) and refuse to participate.
Acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions or negligence but the actions, inactions or negligence of others, the rules of play, or the condition of the premises or of any equipment used. Further, that there may be other risks not known to us or not reasonably foreseeable at this time.
Assume all the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability or death.
Release, waive, discharge and covenant not to sue the United States Ju-Jitsu Federation, its officers, its affiliated clubs, regional sports organizations, their respective administrators, directors, agents, coaches, and other employees of the organization, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessees of premises used to conduct the event, all of which are hereinafter referred to as ‘releasees’ from any and all liability to each of the undersigned, his or her heirs and next of kin for any and all claims, demands, losses or damages on account of injury including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the releasee or otherwise.
