COVID-19 Liability Release Form

In consideration of the agreement of Tempo Rhythmic Gymnastics (TRG) to accept my child(ren) and / or myself as participant in TRG activities, and as a precaution to prevent the spread of the novel coronavirus (COVID-19) and reduce the risk of potential exposure, I the parent or legal guardian of said participant(s) hereby attest that:

1.     If my child or a household member is experiencing cold or flu-like symptoms, or running a fever over 100 F, my child will stay home.

2.     My child and household members currently have no symptoms related to COVID-19 (cough, fever, or shortness of breath).

3.     If my child or a household member visit or travel internationally to a known infected area, my child will refrain from coming to gymnastics class for 14 days upon return.

4.     If my child or a household member has been in direct contact with someone who tested positive for COVID-19, my child will stay home for 14 days.

5.     I understand that my child is allowed to wear a facemask.

6.     My child will wash her/his hands when instructed to do so by TRG’s staff.

7.     I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child and I may be exposed to or infected by COVID-19 by attending TRG classes.

8.     I understand that the risk of becoming exposed to or infected by COVID-19 at TRG may result from the actions, omissions, or negligence of myself and others.

9.     I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury of any kind to my child(ren) or myself.

10.  On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless TRG, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto.

I understand and agree to the terms and policies of Tempo Rhythmic Gymnastics above.
