It's Summer Camp time! Our school offers a camp experience for your child that is equally educational as it is exciting and fun! It is sure to be a safe, positive, and motivational camp adventure where your child is guaranteed to obtain physical, mental, and moral fitness.

Camp will be held from *9am-12:30pm Daily*
Students $279 per week and non Students are $299 per week.

Billing and Payment
Billing and payment processing for Businesses' events is performed by Member Solutions, Inc. ("MSI"), and this contract is specifically assigned to MSI for all such purposes. Customer knowingly and irrevocably authorizes MSI to charge electronically Customer's bank account and/or credit card for any and all amounts due for Customer's, and anyone registering under Customer's, attendance and participation in the Event. Any returned payment will be assessed a $25.00 fee. Any unsuccessful electronic transaction is subject to a $10.00 processing fee. MSI is not responsible for any bank fees incurred by Customer. WAIVER OF RESPONSIBILITY

THE STUDENT AND/OR GUARDIAN hereby represents that the student is in good physical condition, is not suffering from any heart, lung, or other bodily ailment, is in all respects physically fit to engage in the martial arts course, and has fully informed the authorized agent accepting this agreement of any pertinent health information to the contrary.

THE STUDENT AND/OR GUARDIAN has been advised that strict observance of the rules and regulations relative to martial arts self-defense training is mandated.

In recognition of the possibility of accidental injury or death connected with martial arts training,
THE STUDENT AND/OR GUARDIAN waives the right or cause of action of any kind arising as the result of such activity from which any liability may or could accrue to the school, its officers, agents, employees, instructors, students, and/or owner/landlord of the premises.

THE STUDENT AND/OR GUARDIAN understands and agrees that the school, it’s agents, instructors, staff, and landlord will be held harmless for injuries, damages, claims, lawsuits, proceedings, costs, attorney fees, or other losses, etc. not caused by or resulting from the negligence of the owners, operators, employees, or persons in charge of such establishment.

THE STUDENT AND/OR GUARDIAN understands that the student’s tuition is to be made at the time of registration and is not affected by the student’s attendance thereafter. THE STUDENT AND/OR GUARDIAN further understands that failure to complete the camp session does not relieve him/her of his/her obligation to pay the tuition in full.

THE STUDENT AND/OR GUARDIAN will faithfully comply with all the rules and regulations of the school and the traditions of Martial Arts.

THE STUDENT AND/OR GUARDIAN UNDERSTANDS his/her/their rights and obligations as stated above and on the reverse side of the agreement.
