Any returned payment will be assessed a $25.00 fee. Any unsuccessful electronic transaction is subject to a $10.00 processing fee. MSI is not responsible for any bank fees incurred by Customer. I hereby voluntarily submit my attendance and participation in the Holiday Cup Taekwondo Tournament, November 19, 2017 at Highline Community College Pavilion, sponsored by the Seattle Taekwondo Academy LLC, directed by its affiliated member schools and/or clubs and do hereby assume full responsibility for any and all damages, injuries or losses that I may sustain or incur, if any, while attending or participating and I hereby waive all claims against the promoters, operators or sponsors of said Seattle Taekwondo Academy LLC, individually or otherwise for any claim for injuries that I may sustain. I fully understand that any medical treatment given to me will be of first aid treatment type only. House Bill 1824 requires medical clearance of youth athletes suspected of sustaining a concussion, before sending them back in the game, practice or training. Youth athletes who are suspected of sustaining a concussion or head injury be removed from play. Youth athletes who have been removed from play receive written medical clearance prior to returning to play from a licensed health-care provider trained in the evaluation and management of concussion. The organizing Committee reserves the right to suspend or remove me from the property if I do not cooperate with the tournament rules and guidelines without refund. I consent that any pictures furnished by me or any pictures or images of me in connection with the event can be used for publicity, promotion or video showing and I waive all compensation in regard, thereto. This is a private event. Organizing committee has the right to refuse entry or admission. No refund and all financial transactions are final.
