

This "on-line" Dan Grade Application is for MAIF MEMBERS who already have certified Judo Dan Grade Certification from a Recognized Judo Organization and desire also to have their Judo Dan Grade certified as International Dan Grade in TRADITIONAL KODOKAN JUDO by the Martial Arts International Federation (MAIF).
Note: In using this On-Line Registration Form, it is Important to know that you are submitting an "Application" for International Judo Dan Rank Certification,
MAIF-ITKJ reserves the right to decline the certification request. This is only for Black Belt Ranks..
MAIF-ITKJ Yudansha Ranks: Black Belt - $150, Shodan - $175, Nidan - $200, Sandan - $225, Yondan - $250, Godan - $300, Rokudan - $350, Shichidan - $400, Hachidan - $450, Kudan - $500, Judan - $550.
(Note:Life Members pay only a "flat fee” of $150.00 for any Dan Grade level certification or promotion.)
You must be a "Current Annual or Life Member" of MAIF.
You are required to submit the following Documents, Photo and Information to the following Email Address: MAIF.SecretaryGeneral@gmail.com or brucebethers@gmail.com
1. Copies of all your Dan Grades Certificates.
2. Your complete martial arts bio or resume.
3. Your Photo (a "close-up" in Business Attire).
4. Letters of Recommendation.
Applicants upon approval from MAIF-ITKJ will receive the following:
1. MAIF Official Letter notifying you of approved International Judo Dan Grade from MAIF-ITKJ
2. MAIF-ITKJ Dan Grade Certificate
3. MAIF Budo Pass Book
4. MAIF & ITKJ Patches
5. Dan Grade Patch
6. ITKJ Lapel Pin.
NOTE - The NON-refundable Processing Fee for all Judo Dan Grades is $75.00 USD.

Waiver of Liability -
In consideration of being allowed to participate in any way in the Martial Arts International Federation (MAIF) athletics/sports program, and related events and activities, the undersigned:
Agree that the parent(s) or legal guardian(s) will instruct the minor participant that prior to participating, he or she should inspect the facilities and equipment to be used, and if the participant believes anything is unsafe, he or she will immediately advise his or her coach or supervisor of such condition(s) and refuse to participate.
Acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions or negligence but the actions, inactions or negligence of others, the rules of play, or the condition of the premises or of any equipment used. Further, that there may be other risks not known to us or not reasonably foreseeable at this time.
Assume all the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability or death.
Release, waive, discharge and covenant not to sue the Martial Arts International Federation, its officers, its affiliated clubs, regional sports organizations, their respective administrators, directors, agents, coaches, and other employees of the organization, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessees of premises used to conduct the event, all of which are hereinafter referred to as ‘releasees’ from any and all liability to each of the undersigned, his or her heirs and next of kin for any and all claims, demands, losses or damages on account of injury including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the releasee or otherwise.
