Billing and Payment
Billing and payment processing for Businesses' events is performed by Member Solutions, Inc. ("MSI"), and this contract is specifically assigned to MSI for all such purposes. Customer knowingly and irrevocably authorizes MSI to charge electronically Customer's bank account and/or credit card for any and all amounts due for Customer's, and anyone registering under Customer's, attendance and participation in the Event. Please note, you may see all or any portion of the following on your bank or credit card statement: "MSI*PROMARTIALARTS 8882448044 ONLINE REG FEE". Any returned payment will be assessed a $25.00 fee. Any unsuccessful electronic transaction is subject to a $10.00 processing fee. MSI is not responsible for any bank fees incurred by Customer. MSI has the sole right to resubmit returned or declined items (plus applicable fees) without prior notice. BILLING INQUIRIES -- 888-244-8044 or by mail to PO Box 297, Hatboro, PA 19040.

PERMISSION TO PARTICIPATE FORM

PRO Martial Arts Photo/Video Release Form/Authorization to use photos and/or audio/visual material:

I, hereby authorize Pro Martial Arts and/or anyone affiliated with Pro Martial Arts to use, reproduce, and/or publish photographs and/or video that may pertain to me and/or my children including image, likeness, materials, broadcast public
service advertising (PSA’s) or for other related endeavors. This material may also appear on the corporation’s internet webpage/various social media platforms. this authorization is continuous and may only be withdrawn by my specific rescission of this authorization. Consequently, the corporation may publish materials, use my name, photograph and/or make references to me in any manner that the corporation deems appropriate in order to promote the Pro Martial Arts Corporation.

Liability waiver
I hereby agree to allow myself/my child to participate in the Pro Martial Arts Fitness Programs(s) given by Pro Martial Arts, its officers, directors, employees, coordinators, and agents, upon the understanding and agreement that:
1)You represent to us that your child has no physical, mental, or emotional illness that could impair their participation or that could make participation injurious. While every effort will be made on our part to make the events and facilities as safe as possible, you realize that any physical activity has the potential for injury and you waive any claim of accidental and/or negligent tort damage against Pro Martial Arts and/or our principles, officers, or instructors, resulting from the activity.
2)I also release Pro Martial Arts from any liability arising from my use of any and/or all equipment provided by Pro Martial Arts.
3)I understand I will be held responsible for any negligent damage (replacement/repair) to equipment I am using that is owned or provided by Pro Martial Arts. I hereby acknowledge an assumption of risk by accepting and agreeing to allow myself/my child to participate in Pro Martial Arts activities.
