These terms and conditions apply between "the Business" and those persons registering for and participating in the Businesses' events ("Customer").

-RELEASE AND WAIVER OF LIABILITY: Student hereby represents that he/she is physically fit to take the prescribed course of instruction and that he/she has had an opportunity to observe and participate in the above described art of self defense or fitness prior to the signing of this agreement. We, the student & guarantor, on behalf of ourselves, members of our family, our heirs, executors, administrators and assigns, hereby forever release, discharge and hold harmless, Choe’s HapKiDo of Youth, LLC., it’s representatives and agents for any injury, loss, or damage to my person or property howsoever caused, arising out of or in connection with my taking part in martial arts classes and activities and not withstanding that the same may have been contributed to or occasioned by the negligence of Choe’s HapKiDo of Youth LLC, it’s representatives or agents. Student understands that during the course of instruction, employees of Choe’s HapKiDo of Youth LLC and/or other students or authorized persons will be engaged in a course of conduct requiring physical contact; and he/she gives full consent to such contact as is required by training. I further agree that any pictures or video taken of or by me in connection with Choe’s HapKiDo of Youth LLC can be used by Choe’s HapKiDo of Youth LLC and its affiliates for publicity or promotion without compensation at this or any other time.

TERMS: You may cancel this agreement if you relocate your residence further than 30 miles from any school operated by Choe’s HapKiDo. This contract may also be cancelled in a case of death, or if the school ceases operation. If you become temporarily disabled, your liability for payment will proceed under its original terms , but the duration of the original contract will be extended at no cost to you for a period equal to the duration of the temporary disability. If you become permanently disabled, the original agreement will be cancelled. You must prove such disability by a doctor’s certificate, which shall be enclosed with the written notice of disability sent to the billing company, Member Solutions, P.O. Box 297, Hatboro, PA 19040. No prepaid tuition shall be refunded, including your deposit, down payment, registration fee, etc. Choe’s HapKiDo of Youth LLC reserves the right to alter the days, hours of operation and classes if deemed necessary. The student and buyer agree unconditionally to pay the above tuition without regard to the attendance made or classes missed. Due to certain Management Agreement Provisions with our Billing Company, the owner or manager of this school does not have the authority to cancel, put on hold or freeze any student financial agreement, regardless of the reason or circumstances. BILLING AND PAYMENT: Billing and payment processing for Businesses' events is performed by Member Solutions, Inc. ("MSI"), and this contract is specifically assigned to MSI for all such purposes. Customer knowingly and irrevocably authorizes MSI to charge electronically Customer's bank account and/or credit card for any and all amounts due for Customer's, and anyone registering under Customer's, attendance and participation in the Event. The phrase "8882448044 ONLINE REG FEE" or "MSI*CHOESHAPKIDOOFYOUTHL" will appear on your bank or credit card statement. Any returned payment will be assessed a $25.00 fee. Any unsuccessful electronic transaction is subject to a $10.00 processing fee. MSI is not responsible for any bank fees incurred by Customer.
