USMAF DAN GRADE OR DEGREE APPLICATION
This "on-line" Dan Rank Registration is for all Martial Artists who wish to have their martial art rank(s) recognized by and registered with the UNITED STATES MARTIAL ARTS FEDERATION (USMAF), regardless of rank, system, gender, ethnicity or age.
In using this On-Line Registration Form, it is Important to know that you are submitting an Application for Dan Rank Certification, Validation or Promotion.
Note, if your Application for Dan Rank certification of promotion is Not approved, the fee collected will be refunded to you less the processing fee.
For ranks "below" Black Belt - You will need to provide your Sensei's name, phone number and email address so that training and rank can be verified.
For all "Dan Grades" of Black Belt -
You are required to submit the following Documents, Photo and Information to the following Email Address: natlhq.usmaf@gmail.com
1. Copies of all your Dan Grades Certificates.
2. Your complete martial arts bio or resume.
3. A copy of your system's rank requirements for all ranks - both kyu & dan grades (or equivalent).
4. Your Photo (a "close-up" in Business Attire).
5. Letters of recommendation. (minimum of 3 Letters)
6. Complete NCSI On-Line Background Check
7. You are required to attend a National Seminar, be evaluated by the USMAF Nationally Certified Instructor / Examiner, or submit a Video of yourself demonstrating intermediate & advance techniques from your system.
8. You must be a "Current or Life Member" of USMAF.
9. NOTE - The NON-refundable Processing Fee for all Dan Grades in $75.00

The United States (USA) Martial Arts Federation is The Official Coalition of USA National Organizations and is the USA National Member of the Martial Arts International Federation (MAIF), with the purpose of unifying the Martial Arts community in an atmosphere of mutual respect and cooperation. The terms of membership are that each individual member consent to waive the United States Martial Arts Federation (USMAF) from all liabilities as indicated below.
Waiver of Liability -
In consideration of being allowed to participate in any way in the United States Martial Arts Federation (USMAF) athletics/sports program, and related events and activities, the undersigned:
Agree that the parent(s) or legal guardian(s) will instruct the minor participant that prior to participating, he or she should inspect the facilities and equipment to be used, and if the participant believes anything is unsafe, he or she will immediately advise his or her coach or supervisor of such condition(s) and refuse to participate.
Acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions or negligence but the actions, inactions or negligence of others, the rules of play, or the condition of the premises or of any equipment used. Further, that there may be other risks not known to us or not reasonably foreseeable at this time.
Assume all the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability or death.
Release, waive, discharge and covenant not to sue the United States Martial Arts Federation, its officers, its affiliated clubs, regional sports organizations, their respective administrators, directors, agents, coaches, and other employees of the organization, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessees of premises used to conduct the event, all of which are hereinafter referred to as ‘releasees’ from any and all liability to each of the undersigned, his or her heirs and next of kin for any and all claims, demands, losses or damages on account of injury including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the releasee or otherwise.
