The 9th Annual Jamaica Invitational Pro-Am "Annie’s Revenge” returns to the Iberostar Grand Hotel Rose Hall October 29 - November 2, 2014.
 
This premier destination event provides attendees the
opportunity to relax and experience the beauty, excitement and bold spirit of golf in Jamaica. The field will
be open to 40 teams, creating a relaxed
environment for participants and their guests to enjoy 54-holes of friendly competition, fantastic weather and luxurious hospitality.

This form is to be used for all Amateur and Non Golf Guest entries for team's paired with a Celebrity.  A team's spot will be held on a provisional basis once the Professional's entry is submitted or the first Amateur entry is received.  The team entry will be confirmed once all four amateurs' entries are submitted. Team spots will be confirmed on a space-available basis until August 29, 2014 or when the field is full, whichever comes first.

Billing and payment processing for Businesses' events is performed by Member Solutions, Inc. ("MSI"), and this contract is specifically assigned to MSI for all such purposes. Any returned payment will be assessed a $25.00 fee. Any unsuccessful electronic transaction is subject to a $10.00 processing fee. MSI is not responsible for any bank fees incurred by Customer. Would you like us to share information about this event with amateur golfers or Club Professionals you may know?  If so, please type in each person's Name & E-mail Address below.  Any amateur or guest whose referral leads to the registration of a fully paid, new* team will receive a $250 TGC event credit.  The credit can be applied to resort charges at the Iberostar Grand for this year's Jamaica Pro-Am or a future TGC Event Entry.  Please make sure the individuals you're referring place your name in the referral field on their online entry form.    

* New teams are defined as those consisting of participants who have not attended a TGC Destination Pro-Am in the past.  
 
By executing this event entry, I hereby waive any personal or third party claims against The Golf Connection LLC, the Jamaica Invitational Pro-Am and its affiliates, arising out of or resulting from injury, claim, demand, damage, death, or illness caused by participation in, travel to or from, other activities related to this event, or any third party’s gross negligence or willful misconduct.

CANCELLATION TERMS: Any registered player and/or non-golf guest canceling prior to June 29, 2014. Any cancellations after June 29 will receive a full refund less a $300 processing fee. NO REFUNDS will be issued for any player and/or non-golf guest canceling on or after August 29, regardless of whether entries are closed. In either case, the team may fill the canceling player's/guest's spot. If this is done prior to August 29, the $300 processing fee will be waived. Payments for such replacements shall be handled directly between the players/guests.
