Forms/Patterns
Level 1 | Competitors perform basic skills such as blocking, striking and kicking techniques with direction from a coach. |
Level 2 | Competitors perform patterns with assistance from or following along with coach. |
Level 3 | Competitors perform patterns on their own. |
Point Sparring
Level 1 | Competitors have limited mobility of arms and/or legs (do not kick above the waist, do not extend arms fully for hand technique) or have a lower level of cognitive skills. |
Level 2 | Competitors have full mobility of arms and legs (competitor can kick above waist and extend arms fully for hand technique) or have a higher level of cognitive skills. |
Level 3 | Competitors compete from a wheelchair (Sport Wheel Chair required) |

I hereby submit this registration and liability waiver form to participate in the AAU CALIFORNIA STATE TAEKWONDO CHAMPIONSHIPS 2014. I certify that the above information is true and correct and hereby release, discharge and waive any and all responsibility of the State of California, the trustees of the California State University, San Francisco State University (SFSU), Club Taekwondo at SFSU, the Amateur Athletic Union, the California Taekwondo Association, the Academy of Tae Kwon Do, Prof. Bill Dewart, the tournament organizing committee, tournament directors, referees, instructors, coaches, medical staff and other competitors from liability for an injury, including death, and for damage to or loss of property which may be suffered by myself arising out of, or in any way resulting from or attributing in whole or in part to my traveling to, training for, being coached in, using any sports equipment, or participating in the AAU CALIFORNIA STATE TAEKWONDO CHAMPIONSHIPS 2014.
