13/14 Travel Teams Registration

Full Season Travel is offered at the Mite, Squirt, Pee Wee and Bantam age division with two teams typically being placed at each age division. The LRYHA focus is a blend of practice, skill development, and game time to assist the player in reaching their fullest capability. The Full Season Travel program is the core or the LRYHA offering and offers the most ice-time. Upper level Squirt teams participate in the Granite State League and Seacoast Hockey League as do all Pee Wee and Bantam Teams. Mite teams participate in either Granite State or Seacoast League. Our Full Season Travel hockey program provides an environment where players can learn and enjoy hockey in an environment where we stress individual skill, team play, responsibility, hard work, and fun.

LRYHA provides a game jersey and socks. The jersey can only be worn during LRYHA games. Game Jersey�s must be returned at the end of the season.

If this is your first season with a Lakes Region team, you will need to obtain a RELEASE FORM from the last association that you skated with. Your sending association will have to release you from their program, indicated by their signature on the form. Then, you will have to be accepted into our association, indicated by our signature on the form. Once those two signatures have been acquired, the ORIGINAL form must be sent to the VP of the Granite State League for his signature. Once all three signatures are in place, you are then eligible to play in a game as a Laker. *NO EXCEPTIONS* You must also provide a copy of your birth certificate.

Registration Fees

The current year Registration Fee per player is $100.00. The fee will show as an addition to your tuition on the payment page during check out. In addition, the registration fee can be seen at the bottom of this form.

Tuition:

Tuition fees may be divided into monthly payments. Applicants can choose how many payments but all payments must be received no later than November 1st in the given season. Initial payment of 1/2 of the tuition must be received prior to taking the ice. When the player is registered, the credit card will be charged the chosen initial payment and a list of the rest of the payments will be shown. Subsequent payments will be charged according to that schedule. Once a payment date is chosen, the applicant�s credit card will be charged according to the payment schedule each month until final payment is received.There is a $10.00 transaction fee, paid per transaction, for each payment made to defer the cost to LRYHA.

Applicants who fail to meet payment terms will result in that player being removed from the ice.

To provide financial flexibility if you are electing to put a down payment and wish to have the remaining balance be drawn at a later date, please select a date from the drop down box below. Note that payment(s) will be drawn against your account on that date. If you do not select a future date then by default your account will be drawn down upon 30 days from the date of registration, please review this information carefully and select the date that meets your needs.

Please note that 2-3 weeks processing time will be necessary to have the billing company notify you of your new payment schedule as upon conclusion of the checkout, the payment schedule will not reflect the future payment date due to processing issues.

Terms and Conditions:

By proceeding with this registration, you may be waiving certain legal rights, including the right to sue. To: Laconia Ice Arena (Arena), Winnipesaukee Skating Club, Lakes Region Youth Hockey (LRYHA), and their owners, Board Members, Officers, Directors, agents, employees and/or representatives.

Assumption of Risk: I am aware that ice skating, ice skating lessons, ice skating instructions, ice hockey lessons and instructions and playing ice hockey, involves certain inherent risks, dangers and hazards which can result in serious personal injury or death. I am also aware that ice skating rinks and arenas contain dangers that can cause serious injury or death.

I hereby freely agree to assume and accept all known and unknown risks of injury arising out of ice-skating or ice hockey programs/activities.

I recognize and acknowledge that risks of ice-skating and ice hockey can be greatly reduced by: taking lessons, abiding by the Player Code of Conduct and using common sense. Code of Conduct: This code of conduct applies to all parents, player, participants, spectators and anyone in the facility as well as on the premises. Our Zero Tolerance Policy requires all players, participants, coaches, team officials, administrators, parents/spectators to maintain a sportsmanlike and educational atmosphere before, during and after all events. The use of obscene, vulgar or abusive language is strictly prohibited. Any language used in a boisterous manner or inciting unfavorable conduct or language is strictly prohibited. This inappropriate and disruptive behavior can result in expulsion from the property as well as other possible penalties to be determined by the Arena, LRYHA Disciplinary Committee, LRYHA Board and/or League.

Release and Waiver of Claims: For allowing me to participate in ice skating lessons, public skating and ice hockey and ice hockey instructions at the Arena, I Agree, to the fullest extent permitted by law, as follows: TO WAIVER ALL CLAIMS that I have or may have against Laconia Ice Arena, Winnipesaukee Skating Club, Lakes Region Youth Hockey and their owners, Board Members, Officers, Directors, agents, employees and/or representatives arising out of ice skating or ice hockey programs at the Arena or at any Arena used in participation of said program. TO RELEASE the Laconia Ice Arena, Winnipesaukee Skating Club, Lakes Region Youth Hockey and their owners, Board Members, Officers, Directors, agents, employees and/or representatives from all liability for any loss, damage, injury or expense that I (or my next of kin) may suffer, arising out of ice-skating, from any cause whatsoever. Including negligence or breach of contract on the part of the Arena in the operation, supervision, design or maintenance of the Arena. Arbitration: In further consideration of allowing me to participate, I agree to submit any claims arising from ice skating lessons, ice-skating or ice hockey or ice hockey instructions at the Arena to unlimited, binding arbitration before the American Arbitration Association. The arbitration must be commenced within one year after the claim arose. It will be in the town where the Arena is located. Federal Rules of Evidence will govern the proceedings. A court of competent jurisdiction can enforce the arbitration award. Payment Agreement: I agree that I am responsible for the payment of all funds due as specified. I understand and accept the league refund policy as outlined in the LRYHA By Laws and I understand the League is under no obligation to refund or credit any payments of any kind. Placement Agreement: I understand that, as voted by the members, the LRYHA Board is placing players on teams, based upon skill level and that placement decisions are at the sole discretion of the LRYHA Board or its appointed representatives.

I understand these policies are necessary and I am willing to play where the league places me. Binding Effect of Agreement: In the event of my death or incapacity, this agreement shall be effective and binding upon my heirs, next of kin, executors, administrators, assigns and representatives. Severability: If any one or more provisions of this Agreement is held invalid, illegal, or unenforceable, such event will not affect any other provision of this Agreement.

Entire Agreement: In entering into this agreement, I am not relying upon any oral or written representations other than what is set forth in this agreement.

I have read and understand the above agreement and by proceeding with this registration, I understand, acknowledge and agree fully with all policies, procedures and terms of the above stated Participation Agreement, Release of Liability, Code of Conduct, Payment Agreement, Placement Agreement, Waiver of Claims and Assumption of Risk and agree that I am waiving certain legal rights, including the right to sue the arena and league.
