Birthday Party Sign-up

FIVE RINGS TAEKWONDO Ultimate Birthday Party!   Please note: We are having problems with the payment portal for the form. Please enter all your information and select register. Payments can be made via e-transfer to kaleigh@5rings.ca. Join us at FIVE RINGS WATERLOO for the Ultimate Birthday Party experience! You can choose either a Ninja, Laser Tag OR Nerf Wars Birthday Party! All options include a Taekwondo Lesson.    $350 + hst. Party includes 2 hours of fun, Party room set-up (tablecloths, plates, cutlery), Drinks (juice and water), Chips and Loot Bags for up to 12 guests (additional guests $10 each. Maximum of 20 guests). You are welcome to bring in any additional food, snacks, drinks or decorations that you would like!
