KYL Spring 2014

KYL Spring league offers practices, performance training sessions and competitive game play through SLYLA.  Each age group will be grouped into A and B teams to allow each player the maximum experience in both practices and games.

Offered for:

BOYS GRADE:  1/2  3/4  5/6  7/8
GIRLS GRADE: 3/4  5/6  7/8

This registration form is available for BOYS 1/2  3/4 grade and GIRLS 3/4  5/6 grade.  These grades will have 2 practices a week (Tuesday/Thursday) and 1 performance training session a week (Saturday)

1/2 Boys: 5:15pm - 6:30pm
3/4 Boys: 6:15pm - 7:30pm
3/4 Girls: 5:15pm - 6:30pm
5/6 Girls: 5:15pm - 6:30pm

Performance Training
 (Boys/Girls: STL Elite Performance)
Saturdays
1/2 Boys: 10:00am - 11:00am
3/4 Boys: 11:00am - 12:00pm
3/4 Girls: 12:00pm - 1:00pm
5/6 Girls:  1:00pm - 2:00pm

SLYLA Games (10) will begin Sundays (5 weeks):

Billing and Payment
Billing and payment processing for Businesses' events is performed by Member Solutions, Inc. ("MSI"), and this contract is specifically assigned to MSI for all such purposes. Customer knowingly and irrevocably authorizes MSI to charge electronically Customer's bank account and/or credit card for any and all amounts due for Customer's, and anyone registering under Customer's, attendance and participation in the Event. Please note, you may see all or any portion of the following on your bank or credit card statement: "MSI*KIRKWOODYOUTHLAXCLUB 8882448044 ONLINE REG FEE". Any returned payment will be assessed a $25.00 fee. Any unsuccessful electronic transaction is subject to a $10.00 processing fee. MSI is not responsible for any bank fees incurred by Customer. Parent Information
 
We hereby give consent for my son/daughter to participate in practices and games organized by Kirkwood Youth Lacrosse and Missouri Amateur Lacrosse. If we cannot be reached in case of emergency, we also consent for Kirkwood Youth Lacrosse to obtain a physician or hospital of the organization’s choice, if such medical care as is reasonably necessary for the welfare of the player, if he is injured in the course of lacrosse activities, practice or games. Furthermore, I release and hold harmless Kirkwood Youth Lacrosse and its affiliates in the event of any injury sustained to my son through the routine course of practice and game play.

Privacy:

KYL staff WILL NOT sell or share your e-mail addresses or personal information to outside companies. Any personal information you provide will be securely stored and used solely for billing and information purposes.
Your complete privacy is our strict policy.
